Community Based Care Collaboration
Care coordinators work with families who have a child with special health care needs from birth to 21 years. Coordinators promote effective use of medical, educational, and community resources to maximize each child's health potential.
Services provided include:
- Referral to state and local resources
- Advicacy and child/family support
- Communication with teachers, support staff and schools
- Development and implementation of individualized care plans
- Coordinated team meetings between families, physicians, specialists, schools and community partners
- Financial support for medical equipment and health related services not covered by public or private insurance for families who qualify
