Community Based Care Collaboration

Care coordinators work with families who have a child with special health care needs from birth to 21 years. Coordinators promote effective use of medical, educational, and community resources to maximize each child's health potential.

Services provided include:

  • Referral to state and local resources
  • Advicacy and child/family support
  • Communication with teachers, support staff and schools
  • Development and implementation of individualized care plans
  • Coordinated team meetings between families, physicians, specialists, schools and community partners
  • Financial support for medical equipment and health related services not covered by public or private insurance for families who qualify
