Special Medical progams are part of the State of New Hampshire Title V programs for Children with Special Health Care Needs. More than 1200 children are enrolled in one or more of our programs - Child Development Clinic, Community Based Care Coordination, Neuromotor Clinic, and Nutrition Network.
Community Based Care Coordinators work closely with families of children with special health conditions to assist in addressing their uniques and complicated needs such as obtaining medical equipment, appropriate services, coordinating medical visits amongst specialists or attending school meetings.
